Özet

  • İlaç geliştirme süreci uluslararası düzeyde kuralları ortaya konulmuş, ardışık çalışma aşamaları yapılandırılmış bir süreçtir. Klasik araştırmageliştirme çalışmalarının dışında etik yönleri de vardır ve her aşaması “Etik Kurullar” tarafından denetlenir. Molekülün keşfiyle başlayan süreç önce preklinik düzeyde yürütülür. Belli bir veri birikimi sağlandığında başta toksikolojik güvenlilik ve olası etkinlik değerlendirilerek artık faz çalışmalarına geçilir. Faz çalışmaları klinik düzeydeki çalışmalardır ve farklı amaçları olan farklı aşamaları vardır. İnsanla ilaç adayının ilk temasının başladığı Faz I, onu izleyen farklı sayıdaki ve nitelikteki Faz II ve Faz III çalışmalar ruhsat başvurusu öncesinde yürütülür. İlaç ruhsat alırsa klinik araştırmalar bitmez ve Faz IV çalışmalar yapılır. Bu yolla ilacın etkinliğinin ve saptanamamış yan etkilerinin daha büyük kitlelerde incelenmesi sağlanmış olur. Bu süreçte aynı zamanda “farmakovijilans” merkezleri aracılığıyla ilaçların yan etkilerinin kayıtları sistematik biçimde tutulur.Yukarıda özetlenen süreç ne yazık ki bitkisel ürünler için sistematik bir biçimde yoktur. Az sayıda çalışma literatürde bulunur ve bunların çoğu “preklinik” düzeydedir. İlaçlar için yapılanlara benzer nitelikli klinik çalışmalara pek rastlanmamaktadır. Bu nedenle “bitkisel ilaç’’ diye tanımlanmak istenen ve piyasada farklı adlarla da olsa satış izni almış olan bu ürünlere kesinlikle “ilaç” dememek gerekir. İlaç olabilmek için gereken çalışma aşamalarından geçmemişlerdir. Bir molekülü ilaç yapan elde edildiği kaynak değil ilaç geliştirme sürecini yaşayıp yaşamadıklarıdır.

  • Drug development is a process that is structured in consecutive phases and where there are a series of guidelines laid down at international level. It also has specific ethical aspects apart from those in the conventional research-development activities and each phase of drug development is monitored by the “Ethics Committees”. The process, beginning with the discovery of the molecule, is initially performed at preclinical level. Following the accumulation of a certain data, the phase studies begin after the assessment of toxicological safety and potential eff icacy. Phase studies are clinical trials and conducted in various steps for diff erent purposes. Phase I, in which the initial contact between human and candidate drug takes place, a various number of subsequent Phase II and Phase III studies with diff erent features are carried out prior to the application for approval. If the approval was obtained, the clinical trials are not terminated and Phase IV studies are conducted. In this way, the eff icacy and undetected side eff ects of the drug can be tested on larger groups of people. In this process, the side eff ects of the drugs are systematically recorded as well, through the “pharmacovigilance” centers. Unfortunately, there is no such process as outlined above for herbal products. Only a small number of studies, mostly at preclinical level, are available in the literature. The clinical studies for herbal products similar to those for the drugs are rarely conducted. Therefore, these products, which are intended to be considered as “herbal medicines” and obtained the licences to sell in the market, even though the names are diff erent, should absolutely not be referred to as “drugs”. Those products didn’t go through the steps of the research process required for developing a drug. The presence of drug development process makes molecule a drug, not the source, from which it was extracted.
