Uluslararası kuruluşların ortaya koyduğu “etkin devlet” vurgusu, bütçe temelli dönüşümde temel oluşturmaktadır. Etkin devlet modelinde, devletin hizmet sektöründen çekilmesi, piyasa güvenliğini sağlayan bütçeyi sağlamlaştıran politikalar oluşturması istenmektedir. Ortaya konulan ücret rejimi politikaları etkin devlet uygulamaları kapsamında, bütçenin sürdürülebilirliği çerçevesinde ortaya çıkmıştır. Bu çalışmada kamu sağlık hizmetlerinde oluşan bütçe temelli dönüşüm ve döner sermaye bütçesinden yapılan performansa göre ödeme, sabit ödeme ve nöbet icap ödemeleri tartışılmaktadır.

“Eff icient government”, the concept which has been introduced by international organisations lays the basis for budget-based transformation. The model of eff icient government requires the government to withdraw from services sector and to make policies ensuring market safety and reinforcing the budget. Existing wage regime policies came up as a part of eff icient government practices and sustainability of the budget. The present study discusses budget-based transformation in public healthcare services and performance-based payment, fixed payment and shiftbased payments out of revolving fund.
