Özet

  • Bu yazıda Sağlıkta Dönüşüm Programı (SDP) adı verilen sağlık reformu sürecinin çocuk sağlığı üzerine etkileri incelenmiştir. İnceleme SDP’nin çocuk sağlığı göstergeleri ve çocuk sağlığına yönelik sağlık hizmetlerinin değerlendirilmesi üzerinden yapılmıştır. Bu kapsamda çocuk sağlığı göstergelerinden olan Bebek Ölüm Hızı (BÖH) yıllar içinde azalmakla birlikte, postneonatal BÖH, 2013 TNSA verilerine göre bir önceki döneme göre %50 artmıştır. Çocuk sağlığının diğer bir önemli göstergesi olan beslenme durumu değerlendirildiğinde yine beslenmenin özellikle kentlerde ve batıda bozulduğu, bebek ve çocuk izlemlerinin nitelikli yapılmaması dolayısıyla erken dönemde saptanamaması nedeniyle ciddi beslenme bozukluğu boyutunun arttığı saptanmıştır. Bunların yanı sıra TNSA 2013 raporuna göre tam aşılı çocuk yüzdesinin 2008 yılına göre %8 azalarak %74.1’ e düşmesi oldukça önemli bir bulgu olarak karşımıza çıkmaktadır. Diğer önemli bulgu ise hiç aşı olmamışlarda görülen %89 değişim hızına denk gelen artış olmasıdır. Bu bulgular SDP sürecinden çocuk sağlığının olumsuz etkilendiğini gösteren kanıtlar sunmaktadır. Bugüne kadar olduğu gibi, bundan sonra da herkese eşit ama gereksinimi olana daha fazla, genel bütçeden finanse edilen, kamusal hizmet sunumuna dayalı bir sağlık sistemi taleplerimizi yüksek sesle söylemek için daha güçlü gerekçelerimiz vardır.

  • The article examines the eff ects of the process of health reform known as Transformation in Health Programme (THP) on child health. The study is based on THP’s child health indicators and an assessment of existing child healthcare services. In this context, while infant mortality rate (IMR), an important child health indicator, displays a declining trend over years, there is an increase by 50% in post-neonatal IMR over the previous year according to Turkish DHS data for the year 2013. Coming to nutritional status as another important indicator of child health, it is observed that there is a backward trend in child nutrition particularly in urban centres and western parts of the country and there is significant rise in child malnutrition due to the absence of quality child monitoring and thus failure in early identification of nutritional problems. Further, it is an important finding that, according to the 2013 DHS report, the share of fully immunized children declined by 8% and dropped to 74.1% since 2008. Another important finding is that there is 89% increase in the number of children not immunized at all. These findings indicate that child health is negatively aff ected by the THP process. Now we have even stronger case for demanding a healthcare delivery system as a public service funded by the general budget, according equal services to all but more to those who need that.
