Her ikisi de temel sağlık hizmetleri konusunda yayımlanmış iki uluslararası bildirge olmasına karşın, Alma-Ata ve Astana Bildirgeleri amaç ve hedefl er konusunda farklılıklar içermektedir. Alma-Ata Konferansı temel sağlık hizmetleri kavramına odaklanmak, kapsamını belirlemek ve öneriler geliştirmek amacıyla düzenlenmişken, Astana Konferansı temel olarak “Evrensel Sağlık Kapsamı” ve “Sürdürülebilir Kalkınma Hedefl eri” ne odaklanmıştır. Alma-Ata Bildirgesi “herkes için sağlık” çağrısıyla dünyaya sağlık ile ilgili olarak yeni bir izdüşüm sunmaya odaklanmışken, Astana Bildirgesi sağlığı hak olmaktan çıkartan yaklaşımını açık olarak sergilemiş ve evrensel sağlık kapsamı ile neoliberal sağlık reformlarının başat özelliklerinden birisi olan sağlık hizmetlerinin sigorta yolu ile finanse edilmesi yaklaşımını ön plana çıkartmıştır. İnsanlığın, sağlığı gerçekten hak olarak benimseyecek ve sağlık hakkının tüm bileşenlerini herkese sağlayabilecek eşitlikçi küresel bir düzene ve temel sağlık hizmetleri eksenine oturtulmuş kamucu bir sağlık sistemine duyduğu gereksinim sürmektedir.

While both declarations on primary health services have their international character, the declarations of Alma-Ata and Astana have some diff erences with respect to their objectives and targets. The Alma-Ata Conference was organized to focus on the concept of primary health services, delineate its scope and to develop relevant suggestions whereas the Astana Conference basically focused on “Universal Health Coverage” and “Sustainable Development Goals”. While the former focused on presenting a new projection to the world with its call “health for all”, the latter explicitly demonstrated its approach of depriving health from its human right dimension and brought to the fore the approach of financing health services through insurance which is one of the underlying characteristics of neoliberal health reforms. The humanity is still in need of a publicoriented healthcare system based on an equalitarian global order around the axis of primary health services which establishes health as a right and is capable of off ering all components of this right to all.
