Özet

  • Avustralya yaklaşık 25 milyon nüfuslu, federal monarşi ile yönetilen bir ada ülkesidir. Avustralya sağlık sistemi, finansal açıdan federal hükümetçe vergilerle oluşturulan medicare denilen zorunlu sigorta sistemi ve özel sigortalardan oluşan karma sistemle yürütülür. Birinci basamak sağlık hizmetleri ve kamu hastaneleri federal hükümet ve medicare güvencesindedir. Ancak sistem diş, göz ve fizyoterapi gibi tedavi harcamalarını karşılamaz. Bu karma sistemde Avustralyalıların üçte birinin özel sigortası vardır. Liberal Avustralya sistemindeki bu karma sağlık finansmanı hem birinci basamak hem de ikinci basamak sağlık hizmetlerinde görülmektedir. Avustralya’da 25 Ocakta çıkan ilk vaka ve 1 marttaki ilk ölümle başlayan COVID-19 pandemisi iki dalga şeklinde seyretmiştir. Pandemide sınırların kapatılması, işverenlere ve çalışanlara mali destekle esnek çalışma düzeni ve sosyal etkinliklerin önlenmesi ile birinci dalga atlatılmıştır. İkinci dalga önlemlerin gevşetilmesi sonucu ortaya çıkmış, yeniden kısıtlamalar getirilmesiyle kasım itibariyle sönümlenmiştir. Kasım 2020 başında günlük vaka sayıları 5-10 arasında seyretmekte bu durum 25 Nisan 2021 itibarıyla devam etmektedir. 13 Kasım 2020 ‘a dek Avustralya’da toplam vaka sayısı 27.711, iyileşen sayısı 25.311, ölen hasta sayısı ise 907 iken, 30. Nisan 2021 için günlük yeni vaka sayısı 30, ölüm sayısı sıfırdır ve toplam onaylanmış vaka sayısı 29.779, ölüm sayısı 910 dur (.WHO, 2021). Bu durumun uygulanan sınırların ve eyaletler arası sınırların kapatılması dahil ciddi kapatma önlemleri, federal hükümetten bir ölçüde finansal destek sağlanması, önlemlerin yüksek para cezaları ve hapis cezaları ile uygulanmasından ve bir ada ülkesi olmasından kaynaklandığı söylenebilir. Ancak, sağlıkta eşitsizliklerin olduğu ülkede, salgından aborijinler, göçmenler, yabancı öğrenciler gibi gruplar olumsuz etkilenmiş; işsizlik oranları artmış, turizm sektörü etkilenmiş , birçok sanatsal etkinlik askıya alınmıştır. Avustralya Tabipler Birliği rehavete kapılmadan önlemlerin sürdürülmesini önermiştir.

  • Australia is an island country with has a population of approximately 25 million people and is governed with federal monarchy. The Australian health system is managed with a mixed system consisting of the compulsory insurance scheme known as Medicare that is operated with taxes by the Federal Government and private insurance schemes. Primary healthcare services and public hospitals are covered by the Federal Government and Medicare. However, the system does not cover treatment costs in fields such as dentistry, ophthalmology and physiotherapy. In this mixed system, one third of Australians have private insurance. This health financing program in the liberal Australian system is encountered in both primary and secondary healthcare services. In Australia, the COVID-19 pandemic that started with the first care on 25 January and the first death on 1 March progressed in the form of two waves. The first wave was overcome by closing the borders in the pandemic period, bringing a flexible working order and financial support for employees and preventing social activities. The second wave emerged as a result of loosening the precautions, and it diminished by November when restrictions were brought back. From the beginning of November 2020, the number of cases per day had been around 5-10, which still continued by the date of 25 April 2021. While the total number of cases was 27,711, the number of those who recovered was 25,311, and the number of those who lost their lives was 907 until 13 November 2020, for 30 April 2021, the number of new cases per day was 30, the number of deaths was zero, the total number of confirmed cases was 29,779, and the number of total deaths was 910 (WHO, 2021). It may be stated that this situation resulted out of the restrictions that were applied, strict closure precautions including the closure of interstate borders, provision of financial support by the Federal Government, implementation of the precautions in a form enforceable by heavy fines and imprisonment, as well as the fact that Australia is an island country. However, in the country where there are inequalities in health services, groups such as Aborigines, migrants and foreign students have been negatively affected by the pandemic, the unemployment rates have increased, the tourism sector has been affected, and several arts-related activities have been suspended. The Australian Medical Association recommended to continue the precautions without complacency.
